Now that you are booked onto one of our beginners course we need you to download our information form

Please print both pages and fill in the shaded section on the first page and bring it with you to the first lesson.

This will give us all the information we need for insurance purposes, and we will keep it to record the equipment you will be using and your progress during the lessons.
